The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete an online appointment request.

Patient Forms

If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Thank you and please contact our office if you have any questions at all.

Payment Options

For your convenience, we accept cash, personal checks, money orders, and most major credit cards. We also offer CareCredit financing. Payment is expected at the time services are performed. Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.

Our policy requires a minimum payment of half-down at the time of your visit and a paid balance no later than 60 days unless other arrangements have been made with our team.  If your account is not paid within 90 days of the date of service and no financial arrangements have been made, a 21% interested fee will be applied.  You will be responsible for legal fees, collection agency fees, interest charges and any other expenses incurred in collecting your account.  If no payments have been remitted after 6 months, the account will be turned over to Bonneville Collections. 

Dental Insurance

As a courtesy to our patients, we offer a complimentary insurance benefit check prior to your appointment. However, it is ultimately the patient's responsibility to provide accurate and up-to-date insurance information. We are contracted with a range of major dental insurance providers, and our team is happy to help you understand your coverage. If you have any questions about your plan or whether we are in-network, please don’t hesitate to contact us directly.
